Attention all current and former APTC patients.
Do you know of someone who needs physical therapy? Refer them to APTC between July 1 and December 5, 2007 and you could win an IPOD just in time for the holidays.

Here's what to do:

Either...
1) Fill out the referral form below. Include your name (referring person) and phone number, and the name and phone number of the patient you referred to therapy. If they become a patient between July 1 and December 5, 2007, you will receive one entry into the drawing.
2) Fill out the referral form at APTC. The NEW patient can fill out this form on his or her first visit to APTC. Look for the "Referral Contest" box at the front desk. If they start therapy between July 1 and December 5, 2007, you will receive one entry into the drawing.

One entry per person referred. There is no limit to the number of entries, so keep those referrals coming. On December 6, 2007 one name will be drawn from the pool of entries. This person will win an IPOD. The winner agrees to have their photo taken and put into the next APTC patient newsletter.
